Warm Weather Triggers Early Allergy Season - Health News ...

FRIDAY, March 30 (HealthDay News) ? While unseasonably warm weather delights many people, those with allergies may not be as thrilled with the early arrival of spring.

Arriving along with those beautiful blooms is plenty of pollen that has hay-fever sufferers sneezing at least a few weeks sooner than normal.

And, in some areas, not only is the season starting early, but the pollen counts are breaking records. Several days ago, Atlanta?s pollen count reading was 9,369 particles of pollen per cubic meter, which is 55 percent higher than the old record high set in 1999. Normally, anything above 1,500 is considered high in the Atlanta area, according to the American College of Asthma, Allergy and Immunology (ACAAI).

?Tree pollen in some parts of eastern U.S. started in early February, which is about three to four weeks early, and some areas have had record high counts for days and weeks. For the one in four people who has allergies, this is having a cumulative effect. The longer you get exposure, the worse the problem becomes,? explained Dr. James Sublett, an allergist and spokesman for the ACAAI.

He said mold counts haven?t been high yet, but normally in the winter, mold is gone once the ground freezes. ?We?ve had some mold counts in the moderate range here in Louisville, Ky., and because of the nice weather people are outside more. Again, it?s that continuous exposure that makes allergies even worse.?

And, humans aren?t the only ones enjoying the warmer weather. Ticks and mosquitoes that are normally dormant at this time of the year are already active, according to Richard Ostfeld, a senior scientist with the Cary Institute of Ecosystem Studies in Millbrook, N.Y.

?Adult ticks have been active all winter long. The warm winter weather changed their behavior, but so far there?s no evidence that it?s changed their abundance. And, given the mildness of the winter, there could be a better over-winter survival of mosquitoes,? Ostfeld said. Plus, he said, mosquitoes may get a jump-start on breeding with the warm weather.

If you normally use flea or tick treatments on your animals, hopefully you?ve started those treatments. If you haven?t, now is the time, according to Ostfeld. And, it?s important to be vigilant about checking yourself and children when you come in from outdoors for ticks, he said.

As for dealing with allergies, Sublett said the first line of treatment is over-the-counter antihistamines, such as Allegra, Claritin and Zyrtec. There are also nasal spray antihistamines, as well as topical nasal steroids for the treatment of allergies (most of these are available only by prescription). If you have allergies and asthma, Sublett said that montelukast (Singulair) can help treat both those problems. Singulair is only available by prescription.

Sublett said it?s a good idea to visit an allergist so you can find out exactly what you?re allergic to, so that you can take steps to avoid those allergens.

If you?re allergic to pollen, but love to garden, Sublett said you should wear a mask (with a NIOSH N95 rating) to filter the small particles that you?re bound to stir up while gardening or mowing the lawn.

He also recommended keeping your windows closed and using your air conditioner or home heating vent system to filter the air in your home. Sublett said to close the windows in your car and use the recirculated air setting in the car. In addition, be sure to change air conditioning and furnace filters frequently, and use high-efficiency filters with an 11 or 12 rating, he advised.

If you don?t have a heating or air conditioning system that can filter your home?s air, portable in-room air filters can work well. But, Sublett said, be sure to get one with a HEPA (high-efficiency particulate air) filter that?s large enough for the room you need to clean, and let it run often. He advised against the ionizing type of air cleaners as those can put ozone in the air, which isn?t good for those with breathing problems.

The True Cost of Compliance?

Survey reveals that doing the bare minimum is roughly the equivalent of an invitation to financial disaster.

I?ve read a number of security-related surveys this year, and there seems to be a general theme running through them, albeit most likely unintentionally: Enterprises are woefully lacking in the tools needed to provide better security. Or, in the case of a new survey from LockPath, to provide better risk management.

LockPath recently surveyed 175 compliance and risk practitioners and found that 83.6 percent consider their organization?s risk level to be moderate-to-high. Seventy-eight percent said that regulations requiring compliance are increasing and, yet, 26 percent of respondent have no tools or procedures in place to remain current on federal and state compliance regulations.

Below are some of the other figures from the survey, according to LockPath:

Overall, attaining one consolidated view of risks and staying on top of new regulations are viewed as the most challenging compliance tasks. Among respondents, 80 percent say that the ability to ?consolidate, centralize and mine business-critical risk and compliance data? is the most important feature of a compliance solution, yet 46 percent say their current solution/process needs improvement in this area. Similarly, 63 percent say that the ability to ?automatically generate assessments? is very important, but 43 percent say their current solution/process is insufficient in this area. It also appears that despite the well-known risks that come with third parties, the majority of respondents ( 63.2 percent) cannot ensure that their vendors and partners are in compliance with their policies. Interestingly, two-thirds do not track costs associated with compliance.

This is not a problem unique to American companies. A survey released earlier this month by Thomson Reuters interviewed 500 compliance professionals at financial services companies around the world. According to Corporate Compliance Insights:

Eighty-four percent of compliance professionals surveyed expect to handle more regulatory information from regulators and exchanges this year, with nearly half expecting the level to be significantly higher.

However, the article continued:

While regulatory requirements continue to grow, compliance teams are showing signs of resource constraints limiting their ability to perform vital compliance functions. More than a third of compliance professionals say they spend more than an entire working day each week in simply staying up to date with regulatory changes and assessing their impact on the business.

The survey also says the vast majority of compliance teams aren?t reporting back to executive boards on a regular basis, which raises the question of whether or not people at the top of the corporate ladder are getting enough information on compliance issues.

Chris Caldwell, CEO, LockPath, said what may be the understatement of the year:

It is crystal clear that, despite what some see as the maturing of risk management programs in the past several years, an alarming number of businesses remain overwhelmed by the number of regulations with which they must comply. The results of this survey indicate a strong need for a governance, risk and compliance solution that can help companies of all sizes get a grip on compliance with a platform that delivers integrated risk and regulatory intelligence.

Without the tools (or the budgets to implement the tools) risks are going to increase. As the lack of tools and resources is a common theme in the first quarter of 2012, perhaps it is time enterprises take a closer look at why they don?t have the resources to provide better security and risk management and what the cost could be if things don?t improve.

ScienceDaily (Mar. 27, 2012) ? The University of Maryland released details today of the most extensive full face transplant completed to date, including both jaws, teeth, and tongue. The 36-hour operation occurred on March 19-20, 2012 at the R Adams Cowley Shock Trauma Center at the University of Maryland Medical Center and involved a multi-disciplinary team of faculty physicians from the University of Maryland School of Medicine and a team of over 150 nurses and professional staff.

The face transplant, formally called a vascularized composite allograft (VCA), was part of a 72-hour marathon of transplant activity at one of the busiest transplant centers in the world. The family of one anonymous donor generously donated his face and also saved five other lives through the heroic gift of organ donation. Four of these transplants took place over the course of two days at the University of Maryland Medical Center.

The face transplant team was led by Eduardo D. Rodriguez, M.D., D.D.S., associate professor of surgery at the University of Maryland School of Medicine and chief of plastic, reconstructive and maxillofacial surgery at the R Adams Cowley Shock Trauma Center at the University of Maryland Medical Center. Dr. Rodriguez is board-certified in plastic and reconstructive surgery as well as in oral and maxillofacial surgery. This marks the first time in the world that a full face transplant was performed by a team of plastic and reconstructive surgeons with specialized training and expertise in craniofacial surgery and reconstructive microsurgery.

"We utilized innovative surgical practices and computerized techniques to precisely transplant the mid-face, maxilla and mandible including teeth, and a portion of the tongue. In addition, the transplant included all facial soft tissue from the scalp to the neck, including the underlying muscles to enable facial expression, and sensory and motor nerves to restore feeling and function," explains Dr. Rodriquez. "Our goal is to restore function as well as have aesthetically pleasing results."

The face transplant recipient, 37-year-old Richard Lee Norris of Hillsville, Virginia, was injured in 1997 in a gun accident. Since that time, he has undergone multiple life-saving and reconstructive surgeries. Due to the accident, Mr. Norris lost his lips and nose and had limited movement of his mouth. Mr. Norris first came to the University of Maryland Medical Center in 2005 to discuss reconstructive options with Dr. Rodriguez.

Grant funding from the Office of Naval Research (ONR) in the Department of Defense to Dr. Stephen Bartlett has supported the University of Maryland basic and clinical research program in vascularized composite transplantation leading up to and supporting this groundbreaking face transplant. The ONR funds medical research to support military operational medicine and clinical care of returning veterans. In addition to conducting research, the University of Maryland supports military medicine in a variety of ways, including training military medical staff prior to deployment and performing organ transplant surgeries for patients at Walter Reed/Bethesda National Naval Medical Center.

"The future of medicine depends on rapid translation of research and creating high-performing teams. The face transplant is a perfect example of the life-changing options we can provide for our patients when we combine the expertise of our research and clinical teams to pursue procedures that would have seemed unfathomable not so long ago," says E. Albert Reece, M.D., Ph.D., M.B.A., vice president of medical affairs at the University of Maryland and dean of the University of Maryland School of Medicine.

The team of face transplant surgeons benefited greatly from their experience treating high-velocity ballistic facial injuries at the R Adams Cowley Shock Trauma Center at the University of Maryland Medical Center. The team also includes research scientists and physician scientists from the University of Maryland's nationally recognized Division of Transplantation who have been researching ways to reduce rejection of donated organs and minimize the side effects of long-term immunosuppressive use after transplantation.

"A project like the face transplant requires multi-disciplinary collaboration between numerous clinical services and in many ways is very similar to trauma care," says Thomas M. Scalea, M.D., Francis X. Kelly Professor of Trauma Surgery, director, Program in Trauma, University of Maryland School of Medicine, and physician-in-chief, R Adams Cowley Shock Trauma Center. "Because we have an infrastructure built around multi-disciplinary care, it made sense for the facial transplant program to be housed at the Shock Trauma Center in the University of Maryland Medical Center."

The scientific team that includes Drs. Stephen Bartlett, Rolf Barth, and Eduardo Rodriguez focused on the anatomic and immunologic challenges to craniofacial transplantation. This work has been the basis for Dr. Rodriguez and his surgical team's groundbreaking surgical achievement.

"This accomplishment is the culmination of more than 10 years researching the immune system's response to vascular composite allograft transplants," says Stephen T. Bartlett, M.D., Peter Angelos Distinguished Professor and Chair of the Department of Surgery at the University of Maryland School of Medicine and Surgeon-in-Chief at the University of Maryland Medical Center. "Our solid organ transplant immunosuppressive protocol has led to excellent outcomes for our patients and will be part of the long-term care plan for the face transplant patient."

The face transplant team collaborated with the Living Legacy Foundation of Maryland, the organ and tissue donation program serving most of Maryland. The Living Legacy Foundation of Maryland is a non-profit organization that helps facilitate the donation and recovery of human organs and tissues for transplantation and research, and provides public and professional education on organ donation.

An international group of researchers, including those from Moffitt Cancer Center in Tampa, Fla., have published a paper in the March 14 issue of the New England Journal of Medicine reviewing the results of a study that analyzed mutations in 18 genes of 398 patients who had acute myeloid leukemia (AML). They found that several mutated genes predicted improved outcomes when patients with certain gene mutations were given high-dose induction chemotherapy. Their findings suggest that mutational profiling could potentially be used for both risk stratification and also in helping health care providers make therapeutic decisions for some AML patients.

"Previous studies have found that AML is a highly heterogenic disorder," said study co-author Hugo F. Fernandez, a senior member at Moffitt and associate chief of Moffitt's Blood and Marrow Transplantation Division. "Moreover, recent studies have revealed that a number of genetic mutations in AML patients might have prognostic value. The question of the presence of these gene mutations altering outcomes based on current therapy had not been answered to date."

Their paper cites a clinical trial carried out by the Eastern Cooperative Oncology Group (ECOG) in which dose-intensified chemotherapy improved outcomes in two age sets of AML patients. Based on these findings, the research team hypothesized that carrying out mutational analysis of all known molecular alterations occurring in more than 5 percent of patients with AML might allow for the identification of distinct, molecularly defined subgroups of patients who might benefit from dose-intensified chemotherapy.

The laboratory research team subsequently performed a mutational analysis on diagnostic samples from 398 patients enrolled in the ECOG clinical trial they cited and used patients' frozen sample cells for extraction and profiling. The researchers validated the results of this latter group of 104 patients.

"We found that intensification of the dose of anthracycline significantly improved outcomes and overall survival in patients with mutations in DNMT3A, NPM1 or MLL translocations," said Fernandez. "This finding suggests that mutational profiling could be used to determine which AML patients will benefit from dose-intensive induction therapy."

"Most importantly," said Fernandez, "this study demonstrates how integrated mutational profiling of samples from a clinical trial cohort can advance understanding of the biologic characteristics of AML."

Lawyers for Army Staff Sgt. Robert Bales, suspected of killing 17 Afghan civilians, will likely mount a two-pronged defense, military law experts say, attacking the evidence against him while also arguing that his reported combat injuries and mental trauma created diminished mental capacity.

Bales? civilian attorney, John Henry Browne, has suggested such an approach in his public comments on the case, in which the Army has identified the soldier as the lone suspect in the March 11 attack but not yet charged him.


?There?s no forensic evidence, there?s no medical examiner?s evidence, there?s no evidence about how many alleged victims or where those remains are,? he told NBC Nightly News on Tuesday, adding that he intends to travel to Afghanistan to oversee his own investigation.

But he also stated that his client had ?no memory? of the attack and suggested that could be from a concussive head injury. In comments to CBS News on Monday, he indicated he would make a "diminished capacity" argument rather than pursue an insanity defense.

Some military law experts interviewed by msnbc.com said they expect?a legal pincer attack, in which the defense may try to win acquittal by attacking the evidence but have a fallback position aimed at winning a lesser sentence than the death penalty -- which Defense Secretary Leon Panetta has said could be sought in this case.

Military officials have said that after drinking at an Army outpost in southern Afghanistan on March 11, Bales, 38, crept away in the night to two nearby villages, where he shot his victims and set many of them on fire. At least nine of the 17 victims were children, they said.

Gary Solis, former head of the Marine Corps? Military Law Branch and current adjunct professor of law at Georgetown Law School, said the fact that the crime occurred in a combat zone in a distant country complicates the task for prosecutors, who are expected to charge Bales with premeditated murder and other crimes.

To convict Bales of that charge, prosecutors would have to prove that people died, the means by which they died, that the accused is the person who used those means and had premeditated the offense, Solis said.

That would be no easy feat, given the possibility of numerous crime scene complications, he said.

?The prosecution is under additional burden in that they?re trying a crime that happened ? 9,000 miles away,? he said. ?They have no bodies, they have no autopsies, they have no forensics, they have no photographs, they have no witnesses. There is no Afghan who is going to come here to testify against this guy, so how do they prove premeditation? It?s going to be a problem for them.?

Daniel Conway, a lawyer and former Marine staff sergeant who has been involved in battlefield investigations in Iraq and Afghanistan of alleged crimes by U.S. soldiers, said prosecuting Bales will be ?exceptionally difficult.? Even establishing him as the gunman could be problematic, he said.

?It still remains to be seen whether any of these Afghan local nationals can actually identify Bales as the shooter,? he said. ?Now there?s going to be some real linguistic divides here in terms of people?s ? ability to communicate what they saw but you may very well have the potential down the road for a defense that he didn?t do it.?

The physical evidence from combat zone crimes is similarly suspect, Conway said.

?In these combat zone cases, you have crime scenes that are contaminated almost instantly ? bodies are moved, forensic evidence is either contaminated or cleaned, there (are) typically no photographs that are taken of forensic value so you can?t necessarily go back and do a very thorough wound analysis,? he said, noting that it would be difficult to exhume the bodies if they have already been buried due to Islamic tradition.

?It?s not easy to separate the fact from the fiction in this kind of case,? he added.

If Bales? case goes to trial, the defense will have an opportunity to react to the government?s case, because?the Army presents first. That will enable his lawyers to decide whether to focus on attacking the evidence or arguing that Bales? reported combat injuries and mental trauma from the battlefield created diminished mental capacity. Or, they may do both, Solis said.

?The government has to go first and it has to prove its case,? he said. ?He?s going to be ready to take advantage of any chink in the government?s arguments that he perceives in addition to whatever argument he may have.?

Bales was on his fourth tour in a war zone since signing up for the Army after the 9/11 terrorist attacks in 2001. He had spent three years in Iraq on his previous tours, during which time he lost part of a foot and suffered a traumatic brain injury (TBI) due to a vehicle rollover, media reports say. Two days before he allegedly attacked the Afghan villagers, he saw the aftermath of a bombing in which a fellow soldier had his leg blown off, The Associated Press reported.

While an insanity defense remains possible, experts who spoke to msnbc.com note that winning such a case is extremely difficult in a military trial.

Unlike an insanity defense, where Bales would have to be shown not to have known right from wrong to be acquitted, diminished capacity is simply an argument that the crime was not premeditated and that mitigating factors should lessen his punishment.

?That?s very hard, so ? he might have to go with this diminished capacity,? Greg Rinckey, a former attorney with the U.S. Army?s Judge Advocate General Corps who is now managing partner of military law firm Tully Rinckey PLLC, said of an insanity defense. ?Most of the cases that I?ve tried, that?s what we?ve went with is because we couldn?t get to ? the complete no mental responsibility or the capability to stand trial.?

Eugene R. Fidell, who teaches military justice at Yale Law School, said testimony indicating that Bales? was afflicted by post-traumatic stress disorder, or PTSD, could be introduced at this juncture, but would be unlikely to result in acquittal.

?Maybe some psychiatrist will say he suffers from PTSD,? he said. ?That?s not a defense ? probably. There?s no case in which PTSD has given rise to a successful insanity defense in the military.?

Solis said Bales? lawyers would likely put the brain injury, the wounding of his comrade, the multiple deployments and his foot injury into the ?diminished capacity argument box,? with the traumatic brain injury (TBI) possibly being a strong element in support of that claim.

?You can get a doctor who will come in there with a chart and ? show here?s a normal brain and here?s his brain getting TBI,? he said. ?So the jury?s got something concrete ? that they can wrap their not guilty finding around,? if that?s how they?re leaning.

Conway said doctors compare traumatic brain injury to a ?hardware? problem, whereas PTSD is more like a ?software? issue.

Solis, the former head of the Marines law branch, said the horrific nature of the crime could ironically benefit Bales? defense.

?They?re going to say, ?Would somebody in control of their facilities, somebody who didn?t have diminished capacity have done something this wacky??? he said. ?The act itself is inherently supportive of a diminished capacity? argument.

As a result, he said, Bales? case might not even make it to a military courtroom. Perhaps a deal will be struck, or maybe mental health exams -? which could takes months -- will show that Bales is not competent to stand trial.

But Conway, the former Marine who has been involved in high-profile military crime cases, including the 2005 killing by U.S. Marines of 24 unarmed Iraqi men, women and children in Haditha, Iraq, said the defense also runs a risk by telegraphing that it intends to argue diminished capacity.

?It?s a two-edged sword. On the one hand, if they can prevent this from turning into a capital (death penalty) case, that?s a huge victory,? he said. ?On the other hand, they?re giving away the playbook and they don?t have any access to the witnesses. So the government is going to be out talking to everybody trying to rebut the diminished capacity defense.?

At the same time, a defense built on PTSD and brain injury is generally a tough sell in a military courtroom, Conway said.

?We have used it many times? to get charges reduced, he said. ?I can tell you that it?s hard to get a military jury to be sympathetic to these kinds of defenses because the way they look at it is, ?I?ve had multiple deployments, I?ve had multiple concussive events ? I?ve got family problems, and I didn?t go out and do this.??

?So you?re going to have to be able to explain to the jury why this case is different from their own experiences in combat and that?s going to be tough to do.? ?

Lawmakers probe pharmacy ties to price gouging

WASHINGTON (Reuters) - Lawmakers are investigating three shadowy pharmacies in Maryland and North Carolina for diverting critical but scarce drugs from patients to wholesalers, who are then able to resell the medicine at sometimes big markups.

Elijah Cummings, the senior Democrat on the House Committee on Oversight and Government Reform, began a probe in October to discover why certain companies were selling cancer drugs at more than a hundred times their normal cost.

Shortages of hundreds of drugs including cisplatin, a highly effective treatment for testicular cancer, and fluorouracil for colon and other cancers have helped create a lucrative shadow market.

The Food and Drug Administration has said the number of drugs in short supply, which also include anesthesiology and nutrition medications, had risen to 220 in 2011 from 56 in 2006.

According to preliminary details of Cummings' investigation, made public on Wednesday, some wholesalers opened their own sham pharmacies to obtain drugs in short supply and re-sell them. One wholesaler increased the price tenfold from the pharmacy cost.

"It's shocking to the conscience that anyone because of their greed would deny medicines to patients, who are in many instances in critical condition," Cummings said in an interview.

"If it's not illegal, we need to make it illegal."

Prices in the regular supply chain are often set by confidential contracts between manufacturers and distributors, or between drugmakers and group purchasing organizations, which buy medicines on behalf of hospital chains and pharmacy systems.

Secondary wholesalers and others are under no such price constraints, though some say they must buy the drugs at higher prices initially because they have no contract.

President Barack Obama made drug shortages a national priority in October. He directed regulators to report price gouging to the Federal Trade Commission.

Cummings is investigating the price gouging along with Senator John Rockefeller, chairman of the Senate Commerce Committee, and Senator Tom Harkin, chairman of the Senate Committee on Health, Education, Labor, and Pensions.

The inquiry found cases of the pharmacy and wholesaler being headed by the same person, or by a husband and wife pair.

It was not immediately clear where the pharmacies bought the drugs in the first place, and lawmakers did not yet know how much money they might have made off the practice.

Lawmakers said the investigation continues and they are examining at least 19 other pharmacies.

BACK CORNER PHARMACY

In one situation described by the lawmakers, Jessica Hoppe, the president of a drug wholesaler called International Pharmaceuticals Inc, applied to get a pharmacy license to sell drugs to long-term care facilities and infusion clinics.

But during a routine annual inspection, state regulators discovered Hoppe was operating the LTC Pharmacy from part of her wholesaler office in Durham, North Carolina.

Instead of serving patients, she was funneling drugs like fluorouracil to her wholesale business within a day or two of purchasing them for the pharmacy, documents show.

A pharmacist hired to run the business surrendered her license in September, and North Carolina officials declined to renew International Pharmaceuticals' wholesaler license at the end of 2011. Under state law, it is illegal for pharmacies to sell the bulk of their drugs to wholesalers without a license, and for wholesalers to buy drugs from unlicensed sellers.

It is unclear whether International Pharmaceuticals can still distribute drugs in 23 other states where it had a license. A person describing herself as Jessica picked up the phone at the number listed for the company, and declined to comment about the case.

NEW CROP OF SMALL PLAYERS

Most of the nation's drug supply passes from manufacturers like Hospira Inc and Teva Pharmaceuticals to three leading wholesalers - AmerisourceBergen Corp, Cardinal Health Inc and McKesson Corp - who distribute them to doctors, hospitals, clinics and pharmacies.

Smaller distributors help fill in the gaps in areas where larger companies may not operate or cannot meet the full demand. In recent years, a new crop of small players has surfaced, as the problem of drug shortages has worsened.

"Any time there's a severe shortage of some critically needed good, invariably there are going to be folks who seek to exploit that," said Jay Campbell, executive director of the North Carolina Board of Pharmacy.

Many states allow pharmacies to sell up to five percent of their supply to other distributors in rare or emergency circumstances.

The lawmakers on Wednesday sent letters to three pharmacies and wholesalers in North Carolina, Maryland and New Jersey, asking them to reply by April 11 with detailed records about their operations and profit, and if any manufacturers had authorized them to sell the drugs.

In one of the cases, a pharmacy and wholesaler in Columbia, Maryland appeared to be owned by the same person, Brenda Marshall.

No one returned messages left at the listed number for the pharmacy, Columbia Med Services. The listed wholesaler number, for Columbia Medical Distributors, appeared to be disconnected.

In another case, the lawmakers said, a woman who was the president of a pharmacy called Priority Healthcare LLC in Elkton, Maryland, appeared to pass drugs directly to her husband's wholesale business, Tri-Med America LLC, in Clifton, New Jersey. The woman did not return a call to Priority Healthcare seeking comment, and the number for the wholesaler could not be immediately located.

According to documents from the investigation, Priority Healthcare bought fluorouracil, the cancer drug, for $6.77 a vial, then sold it to Tri-Med America for $50 a vial. The wholesaler later sold it to another company for $69 a vial.

Maryland fined the husband and wife pair $2,000 and asked them to surrender their license by the end of 2011 if they were not planning to open a real pharmacy. It is unclear whether they are still operating.

Maryland recently passed a law that sets a 60-day limit for how long a pharmacy can keep a license without operating, which will take effect in July.

Unhappy public not sure who to blame for high gas

LOS ANGELES (AP) ? Families canceling vacations. Fishermen watching their profits burn up along with their boats' gasoline. Drivers buying only a few gallons of gas at a time because they can't afford to fill the tank.

From all corners of the country, Americans are irritated these days by record-high fuel prices that have soared above $4 a gallon in some states and could top $5 by summer. And the cost is becoming a political issue just as the presidential campaign kicks into high gear.

Some blame President Barack Obama. Some just cite "the government," while others believe it's the work of big, greedy oil companies. No matter who is responsible, almost everyone seems to want the government to do something, even if people aren't sure what, exactly, it should or can do.

A Gallup poll this month found 85 percent of U.S. adults believe the president and Congress "should take immediate actions to try to control the rising price of gas." An Associated Press-GfK poll last month showed 71 percent believe gas prices are a "very" or "extremely" important matter.

Chris Kaufman, who spends $120 a week on gas to travel the 60 miles between his two jobs, at the University of South Dakota in Sioux Falls and at a hotel in Vermillion, S.D., blames the price spike on threats from Iran to cut off oil shipments through the Strait of Hormuz.

"I think the candidates running for president need to take a good hard look at that and determine what their foreign policy is going to be for countries that threaten to do that," he said. "It's going to affect every single citizen in the United States."

Still, he believes the president has little control over gas prices, adding that it is commodities traders who really dictate prices.

Trucker Cory Nissen of Ruther Glen, Va., agrees.

"The president is nothing but a fall guy," Nissen said as he took a break from his rig at a stop in Wilton, N.Y., earlier this week.

Nissen, who is paid by the mile, said he has seen his paychecks shrink because his employer has cut back delivery runs in reaction to the rising cost of fuel. "It needs to change and change quick," he said. "I got bills I got to pay, and half the time I can't pay them."

On the presidential campaign trail, Mitt Romney called on Obama last weekend to fire his energy secretary, interior secretary and Environmental Protection Agency administrator, dubbing them "the gas-hike trio." Fellow Republican Newt Gingrich promised to roll the price of gas back to $2.50 a gallon if he is elected.

Obama mocked Gingrich's promise, saying, "They start acting like they've got a magic wand and will give you cheap gas forever if you elect us."

Amy Lis of Buffalo, N.Y., and her boyfriend canceled their vacation to Florida this spring in favor of a three-hour drive to Cleveland for an overnight stay and a visit to the Rock and Roll Hall of Fame. Even that trip will cost more than $100 in gas.

"It's more than our hotel," she said as she filled up her boyfriend's Ford Ranger pickup.

In truth, there is not a lot the president and Congress can do in the short term to push down gasoline prices. They are tied to oil prices, which have climbed in recent months, pushed by increased consumption from developing nations in Asia, Latin America and the Middle East and by concerns about supply disruptions in Iran and elsewhere.

Mike Siroub, who has operated a Union Oil station in the Los Angeles suburb of Arcadia for 25 years, said customers who used to fill up their tanks now put in just $10 or $20 at a time, telling him that that's all they can afford and that they are driving less or using more fuel-efficient cars.

He himself has joined them.

"I used to have a car with a big V-8 engine," he said. "I traded it for a four-cylinder Toyota Camry."

Among the things the government can do to bring prices down is reduce gasoline taxes or push to get more fuel-efficient cars on the road. The first new fuel standards since 1990 are just now going into effect, and the U.S. auto fleet is now more efficient than ever

People are still feeling the pain.

"When I go out to change the prices, they honk their horns and yell at me," said Siroub whose station's cheapest grade of gas, regular unleaded, was selling for $4.44 a gallon earlier this week. "The other day one person even gave me the finger."

In New York City, some cab drivers say the high cost of gas is prompting them to race through the streets of Manhattan even more recklessly than usual to pick up more passengers during a shift.

"When the gas is up, the money you make is going down," said Less Sylla, who paid $4.17 a gallon earlier this week. "You see a lot of drivers, they're driving, boom-boom-boom, because the lease is too high and it's working on their minds. So that's why they go like that, and it causes a lot of accidents."

Sylla, who said he will vote for Obama, blames greedy oil companies.

In Anchorage, Alaska, general contractor W.M. Lewis said he has had to raises his prices to keep his half-dozen trucks running. "It affects your bottom-line pricing," he said as he put $90.13 worth of gas, at $4.25 a gallon, into one of those trucks.

Milton Walker Jr., whose Louisiana tour company takes vacationers on boat rides through the alligator-infested swamps, said he raised prices last year because of the increased cost of fuel and will do it again if gas hits $5 a gallon. He blames the Federal Reserve, saying it hasn't kept inflation in check.

"I don't think it matters who's president," he said.

Shrimpers in Louisiana and lobstermen in Maine complain that high fuel prices are cutting into their profits. Craig Rogers, who burns through 50 gallons of gas a day tending his lobster traps along Maine's rocky coast, blames commodities traders, though he questions whether politicians are doing enough. He said politicians are too well off to really grasp what ordinary people are going through.

"They can say they feel for us, they can say they understand us, but when you have that kind of money, there's no way you can truly understand what we're feeling," he said.

The Dangers of Anorexia Nervosa | Health and Fitness

Eating disorders are a very serious problem that affects many women. One of the most dangerous of eating disorders is known as anorexia nervosa. Approximately 7 percent of American women suffer from this condition. This is a psychologically based disorder that makes a person perceive themselves as overweight when they are in fact malnourished and extremely thin. Many teenage girls are affected by anorexia nervosa due to the media culture that pushes the concept of extreme thinness as attractive for young girls. Our culture tends to portray beauty by size. Girls are very impressionable at this age and will go to great lengths to achieve what they believe is beauty. Many adult women also battle with this disorder. Oftentimes these individuals feel powerless and by controlling their calories they feel they have control over their lives and extreme willpower.

Causes of the disorder: There are a variety of causes for this disorder. Researchers have found that anorexia usually develops in families where one person has already become anorexic. It seems that environmental factors play a role in anorexia. For example, if a sibling is anorexic it is more likely that another family member may develop it. This may also be related to family dynamics such as tendencies to be overemotional and perfectionists. Anorexia therefore can be a learned behavior that is then modeled by other family members. Cultural influences: Our media is relentless in its portrayal of the ideal woman as being very thin. The image is contradictory to the natural female shape and it is very difficult if not impossible for most women to achieve that level of thinness. It is also extremely unhealthy. The models in fashion magazines have figures that are extremely underweight and these are the role models that teens and women look at when judging themselves and their own bodies.

Behavioral symptoms of anorexia nervosa: This disorder can be deadly. If you are aware of someone who has this eating disorder they need to have immediate medical intervention because it can be life saving for them. Believing that they are overweight these individuals refuse to eat and are terrified at the thought of gaining any weight at all. Women and girls who have this disorder will exhibit an extreme dislike of their own physical body. They are ironically, obsessed with food. They will think and talk about food all the time in a negative fashion. They will compulsively plan out their eating activities. Sufferers also may display very competitive behavior in other areas of their life. They will have perfectionist tendencies as they are very unhappy with themselves and feel that they are not good enough. As the condition worsens and their bodies weaken they may develop mood disorders from lack of nutrition. Depression, mood swings and difficulty sleeping may occur. Physical symptoms include extreme weight loss, dry skin, tooth loss, constipation, and lack of menstruation and lowered heart rate. These physical symptoms can become a very dangerous health concern.

Diagnosis: A medical professional will conduct a variety of tests to determine if the condition is anorexia. This will include a battery of psychological testing as well as physical exams, blood and urine tests. Intervention: If you believe that your loved one or a friend suffers with anorexia you need to intervene and provide them with the support they need to overcome this dreadful condition. The best approach is a calm approach because the individual is suffering from a psychological disorder and may feel that you are attacking them and that what you are saying is not true. Try not to judge them harshly but rather be kind and supportive. Provide them with a safe environment where they can freely discuss their feelings. They may deny that there is any problem. However some people may respond at this point and open up about hurts or reasons why they are unhappy with their weight. This is a very helpful first step. You do not want to alienate them and make the problem even worse as it is a delicate situation to deal with.

Treatment Centers: There are many reputable treatment centers that deal specifically with this disorder. This is where medical doctors and trained counselors can help the individual dealing with anorexia. Getting them into a good program is essential to their well being. They need to be under a doctor?s care to ensure that their physical health does not deteriorate. Psychological counseling as well as possible medication can become part of an effective treatment program. Throughout this process, the person should continue to receive as much support and encouragement from family and friends as possible. This is a battle that can end in victory or defeat. It is a very serious condition and it may be something that the person may have to deal with for the rest of their lives.
